Only once, and never again... well in some countries they like UPS are licenced duty collectors so they do levy whatever applicable taxes on their customers. can be a nasty surprise for some people. on a serious note it's a very sad occurrence. and a very strange looking accident. from that one piece of footage it certainly looks like either mechanical failure or wind shear. poor crew.
